I hereby declare that all the information provided in the application form is true and correct to the best of my knowledge and belief. I also undertake to provide all supporting documents if selected for nomination. The Organizer has the right to refuse my nomination if any information in the documents submitted is found to be untrue. I also agree to abide by the policies, rules, regulations, and decisions of the organizer. In addition, I hereby consent the rights to use my name, voice, image, photograph, and biography in any advertising, publicity, or promotional materials in connection with the Program. Therefore, I shall refrain from any legal action and decision regarding the rights of ownership of these materials.

1. Accuracy of Information
All information provided in the application must be true and correct to the best of the applicant's knowledge and belief.

2. Supporting Documents
Applicants undertake to provide all supporting documents if selected for nomination. The required documents listed in the official MIE Form 2026 include the completed official form, payment proof, curriculum vitae or resume, personal biography, a copy of IC or passport, an official portrait photo, and relevant certificates or supporting documents.

3. Right of Refusal
The Organizer has the right to refuse a nomination if any information or document submitted is found to be untrue.

4. Compliance
Applicants agree to abide by the policies, rules, regulations, and decisions of the Organizer.

5. Administrative Fee
The official MIE Form 2026 states an administrative fee of RM400, subject to the payment instructions and confirmation issued by the MIE committee.

6. Use of Name and Image
Applicants consent to the use of their name, voice, image, photograph, and biography in advertising, publicity, or promotional materials connected with the programme.
